For Referring Providers

Thank you for considering Oak Cliff Physical Therapy for your patient's care.

Oak Cliff Physical Therapy is an outpatient, cash-pay physical therapy practice specializing in orthopedic and pelvic health physical therapy for adults. We do not participate with commercial insurance or Medicare. Patients pay at the time of service and may request an itemized superbill to submit for potential out-of-network reimbursement if their plan includes those benefits.

Appropriate Referrals

We commonly evaluate and treat:

Pelvic Health

Pelvic pain
Urinary urgency, frequency, and incontinence
Bowel dysfunction and constipation
Pelvic organ prolapse
Pain with intercourse (dyspareunia)
Pregnancy and postpartum musculoskeletal conditions

Orthopedic

Neck pain
Back pain
Hip pain
Knee pain
Foot and ankle pain
Shoulder pain
Running and sports-related injuries
Persistent musculoskeletal pain

Please note: Oak Cliff Physical Therapy treats adults only and does not provide pediatric, adolescent, neurologic, cardiopulmonary, home health, or inpatient physical therapy services.

How to Refer

A referral may simply state:

Physical Therapy Evaluation and Treat

Please include any relevant diagnosis, post-operative precautions, or medical restrictions, if applicable.

Option 1: Submit an online referral.

Option 2: Download and fax a signed referral form.

What Happens Next?

We review each referral to ensure it is appropriate for our practice.
If the referral aligns with our services, we will contact the patient, or the patient may schedule their initial evaluation online.
We communicate with referring providers as appropriate regarding the patient's plan of care and progress.
